New Patient Registration
You can register with the practice by providing us with your medical card, if you do not have a medical card the receptionist will give you a GMS1 form to complete.
You will then be made an appointment to see the nurse for a health check. This initial consultation is very important as it enables us to obtain a full medical history from you and carry out routine screening. This information obtained not only helps us care for you, but identifies any special needs you may have. Without this consultation we would experience difficulty in dealing with any administrative work relating to you i.e. provision of repeat prescriptions or medical certificates, or completion of social security claims or insurance reports etc.
Medical treatment is available from the date of registration. Please contact reception for further information.
